"Reminder" Healthcare Campaign

Children diagnosed with chronic health conditions at a young age were approximately twice as likely to present with a mental health disorder. Can we create a tool that helps prevent mental health issues from rising among diagnoses and provide information and resources for both parent(s) and child?
“"Reminder" is a healthcare campaign where both parents and children can find resources and connect with others with similar experiences. "Reminder" is created as a place where everyone can be connected, reminded, and learn to seek assistance and support at any point in their personal healthcare journey. The goal was to create a positive and safe space to emphasize that a child is more than their condition, it is a part of you but it will never define you. The design of the campaign was for the resources to be easily accessible both online through the Reminder app and in print. With my interest in human-centered/healthcare design and experience growing up as a patient in the nephrology clinic at SickKids I set out to design a tool that I would have benefited from at a young age. They are similar tools already set in place but I wanted to design a system that would allow access to said resources at any point in time. By displaying the contrasting sides of a child growing up with a chronic illness through illustration, I wanted to highlight that they are more than their condition and that it doesn't define you as an individual.
