Exploring and mapping a new approach to health information sharing for structurally marginalized populations. The brief provided by C-CHANGE noted that many of those most at risk for developing cardiovascular disease also have disproportionately low access to primary care providers. This gap in continuous primary care and monitoring means that the information and monitoring function of the C-CHANGE guidelines can not be properly fulfilled.
