Exploring and mapping the complexities of patient information transfer in the Ontario. Mapping the flow of patient health information through our healthcare system revealed that siloed information combined with a lack of comprehensive and reliable transfer of patient health information between providers and settings places an undue burden on patients in Ontario’s health system. In order to receive optimal care, patients are relied upon to recall and relay consistent and pertinent information at healthcare service points while simultaneously enduring the stress and discomfort of illness and without easy access to their own records.
